    This form should be used to verify your dependency status for financial aid purposes. You will need to specify your situation, plus provide supporting documentation. If you do not meet any of the criteria outlined, parental information will be required on your FAFSA.
  • You should check this box if you had no living parent (biological or adoptive) at any time since you turned age 13, even if you are now adopted. If you checked this box, please upload a copy of each parent’s death certificate.

  • You should check this box if you were in foster care at any time since you turned 13, even if you are no longer in foster care as of today. If you checked this box, please upload a copy of the court documentation showing foster care status. 

  • You should check this box if you were a dependent or ward of the court at any time since you turned age 13, even if you are no longer a dependent or ward of the court as of today. If you checked this box, please upload a copy of the court documentation showing ward of court status.

  • You should check this box if were in official legal guardianship determind by a court. Please upload the court determination made by your state of legal residence. 

  • You should check this box if are an emancipated minor determind by a court. Please upload the court determination made by your state of legal residence.

  • You should check this box if your school district, a director of a homeless shelter funded by the U.S.Department of Housing and Urban Development, or a director of a runaway or homeless youth basic center/transitional living program determined that you were an unaccompanied youth who was homeless. Upload a letter from a school administrator or director. If you do not a letter, please contact the Financial Aid Office. 

  • In some situations, and with sufficiant documentation, a dependency override may be awarded. According to Federal Regulations, the following conditions DO NOT merit a dependency override though.

    • Parents refusal to contribute to the student’s education
    • Parents unwillingness to provide information on the application or for verification
    • Parents not claiming the student as a dependent for income tax purposes
    • Student demonstrating total self-sufficiency

    If you believe your extenuating circumstance warrants a dependency review, you must submit a personal statement explaining your situation (below). In addition, you should upload at least three letters from a professional source (ex. school administrator, attorney, case worker, police officer, etc.) supporting your claim, plus any other documentation you feel would assist the financial aid office in reviewing your case.
