I understand that if I am selected to attend this Forever A Warrior Program that I will need to arrive at My Warrior's Place by no later then 6pm on Thursday and will attend all workshops and events provided by the program through Sunday at 12pm. I understand and agree that it is my finacial responsiblity for my transportation to and from My Warrior's Place. I also understand that My Warrior's Place will provide housing, meals and program materials while I participate in the program at no cost to me if I am selected for the Forever A Warrior Program. I understand that upon submitting this application, I will be contacted and interviewed by one of the program directors to determine if program is a fit for me.
